Injuries to Members
If you are injured whist engaged in surfclub duties please note that there is Insurance in place to assist with any Medical Expenses associated with the Injury. (Anyone that is injured should tell the doctor that it was on WorkCover. This means they should not have to pay the doctor/medical center.)
The process to recover access this Insurance is as follows.
- A WorkCover Authority Claim Form needs to be completed. It can be found on the SLSNSW website at this link. Click Here for Link
- The incident needs to be logged in the Incident Log Book at the club and reported to the Club Captain at peter.tomkins@aonbenfield.com
- The incident will then to be logged onto SurfGuard (SLSA Computer System by the Club Captain). (Notes for Club Captain - After you have logged in look at the top line of buttons find "Application" select IRD in drop down menu. (A new line of buttons should show) Then "Add a new Incident" - Complete the (very long) form and Add Incident. I’m not sure if you get the opportunity to print it out or if you get an Incident Report number? )
- After complete the WorkCover Authority Form and the Incident Report on SurfGuard send them both to:
